Gut Health: What the Evidence Actually Supports
Gut health has become a marketing category, which makes the actual evidence harder to find. Strip away the supplement claims and what's left is genuinely useful: the bacteria in your large intestine ferment the fibre you eat, and the compounds they produce influence appetite, inflammation, blood glucose and immune function. The way you feed them is mostly boring and mostly free.
Here's what the evidence actually supports, and what it doesn't.
What actually moves the needle
Three things, in order of impact:
- Plant diversity. Different bacterial species prefer different fibres. Eating 30 different plant foods across a week is associated with a more diverse microbiome than eating a large quantity of the same three vegetables. This is the single most actionable finding in the field.
- Total fibre. Diversity without volume doesn't feed much. See how much fibre you need for the Australian targets.
- Fermented foods. Regular intake of live-culture foods increases microbial diversity and has been shown to reduce inflammatory markers.
The 30-plants rule counts more than you think. Herbs, spices, nuts, seeds, wholegrains and legumes all count, and different varieties count separately — red and green capsicum are two. Most people who tally an honest week land somewhere between 12 and 20, which makes the target reachable rather than aspirational.
Why fibre fermentation matters
When gut bacteria ferment fibre and resistant starch, they produce short-chain fatty acids — mainly butyrate, propionate and acetate. Butyrate is the primary fuel for the cells lining your colon and supports the integrity of the gut barrier. Propionate and acetate reach the bloodstream and influence appetite signalling and glucose metabolism.
This is the mechanism behind a lot of otherwise unrelated findings: why high fibre intake tracks with better blood glucose control, lower cholesterol and better satiety. You're not just eating fibre, you're paying bacteria to manufacture useful compounds.
Prebiotics, probiotics and what the difference is
- Prebiotics are the fibres that feed your existing bacteria — inulin and fructans in onion, garlic, leek and asparagus; galacto-oligosaccharides in legumes; resistant starch in cooked-and-cooled potato, rice and slightly green bananas.
- Probiotics are live bacteria you swallow. Effects are strain-specific, which is the part supplement marketing tends to skip — a strain proven for antibiotic-associated diarrhoea tells you nothing about bloating or mood.
- Fermented foods deliver live cultures plus the fibre and compounds produced during fermentation. Yoghurt with live cultures, kefir, sauerkraut, kimchi, miso and tempeh all qualify.
Practically: food-based fermented sources are the better default. Reach for a specific probiotic strain when there's a specific evidenced reason, not as general insurance.

What works against you
The evidence here is less dramatic than headlines suggest, but a few patterns are consistent:
- Very low fibre intake. Bacteria that can't find fibre begin fermenting the protective mucus layer instead.
- Diets dominated by ultra-processed foods, which correlate with lower diversity — largely because of what they displace.
- Antibiotics, which reduce diversity for weeks to months. Necessary when they're necessary; worth rebuilding intake afterwards.
- Chronic stress and poor sleep, both of which affect gut motility and barrier function. This is not a fringe claim — the gut is heavily innervated and responds to stress hormones directly.
- Alcohol at higher intakes, which affects both barrier integrity and composition.
If you're bloated most days
Bloating is the symptom that sends most people looking for gut-health advice, and generic "eat more fibre" is sometimes exactly the wrong answer. Work through it in order:
- Rule out the speed of change. If you recently increased fibre, slow down and rebuild gradually.
- Check the obvious triggers — sugar alcohols in "sugar-free" products, large legume portions introduced abruptly, carbonated drinks, and eating quickly.
- See your GP before trying elimination diets. Coeliac disease must be tested for while you're still eating gluten — cutting it out first makes the test unreliable. Persistent bloating with weight loss, bleeding or anaemia needs investigation, not a diet.
- A low-FODMAP diet works for many people with IBS, but it's restrictive, temporary by design, and needs a structured reintroduction phase. Do it with an Accredited Practising Dietitian, not from a blog.
Bloating alongside unexplained weight loss, blood in your stool, ongoing vomiting, fever, or a change in bowel habit lasting more than a couple of weeks warrants a GP visit rather than a dietary experiment.

Count your plants for one honest week — most people are surprised, and the number itself tends to drive the change. Then add one fermented food daily and one legume-based meal, and let total fibre climb gradually from there.
